Mid Essex Clinical Commissioning Group is asking local people to help support friends, relatives and neighbours when they are ready to come home from hospital.
Dr Donald McGeachy, Medical Director of Mid Essex Clinical Commissioning Group, explained: “Lots of people need that extra bit of care and support to help them stay well at home after a spell in hospital.
“We have a wide range of local services that can help provide clinical support but there’s lots of very practical things people can do in their communities to help our elderly and more vulnerable.”
Some of the top things people can do to help people home from hospital are:
- Make sure they’re stocked up with food basics in case they can’t go out; check their home is warm enough – at least 18 degrees centigrade
- Make sure they understand what to do with any medication they’re taking and if they are running low, help them to organise any repeat prescriptions
- Encourage them to write down any questions they may have about their condition or medication ready for when they next see a healthcare professional
- Make sure they know what to do if they start to feel ill and who to contact for help – NHS111 is available 24-hours a day, 7 days a week to offer advice and access to services if they need them.
