We know that the population in mid Essex is growing at a higher rate than the national average; that our population is ageing and more people have long-term illness.
That’s why we want to continue working with our partners and providers across health, social care, housing and the voluntary sector to support people when they need it the most.
To help you stay well in mid Essex, we are focusing on developing plans that will create services to follow phases of life:

Helping you make healthy lifestyle choices and access to services when you need it.
Supporting our residents with long term illnesses and planning services that help you continue to stay healthly – both mentally and physically.
Live healthily and happily for longer. Allowing you to plan and co-ordinate your care to remain independent for as long as possible.
Helping you to make choices about the end of life and experience a dignified death.
All of these are supported and connected by Connect Well – the social prescribing initiative for mid Essex.
